Request a Fair Hearing
Note: For security purposes, you have 15 minutes to complete this form, otherwise your request will not be received and you will need to start over. You must have javascript enabled in order to use the online forms.
To request a fair hearing you may submit a request online using the E-Request Form.
You may also print out and complete the Fair Hearing Request Form and send via fax or mail to the address indicated on the form. To preserve any aid-continuing rights, be sure to fax or postmark your request as directed on the Notice of Intent.
Once the fair hearing request is processed, you will receive notification (OAH-4420 Acknowledgement of a Fair Hearing Request) via US mail.
Request an Adjournment or Reopening of a Fair Hearing
You may call 877-209-1134 to request an adjournment.

WRITTEN REQUESTS FOR ADJOURNMENTS SUBMITTED LESS THAN SEVEN DAYS PRIOR TO THE HEARING MAY NOT ALLOW SUFFICIENT TIME FOR PROPER PROCESSING AND WRITTEN RESPONSE SHOULD IT BE NECESSARY.
To request an Adjournment of a fair hearing you may submit a request online using the E-Adjournment Form.
You may also print out and complete the Request to Adjourn/Reopen a Fair Hearing Form and send as follows:
- If you are requesting an adjournment, it must be sent via FAX only to the fax number indicated on the form. An adjournment request should not be mailed, as it is less likely to be received before the date of the hearing.
- If you were unable to attend your fair hearing on the scheduled date, and are requesting that the fair hearing be reopened, it can be faxed or mailed to the fax number or address indicated on the form.
Request for Compliance on a Fair Hearing Decision
Use this form only if you have a fair hearing decision.

To report non-compliance with a fair hearing decision you may submit a request online using the E-Compliance Form.
